Clinician Loan Agreement

This agreement is made between Forbes Rehab Services, Inc. (dba Forbes AAC) and you (the clinician). By accepting the terms and conditions of this agreement and providing your ASHA certification number or professional licensure number, you confirm your identity and agree to the terms and responsibilities associated with the loan of Forbes AAC equipment.


Category 1: Clinician Loan Devices
These devices are loaned directly to licensed clinicians for professional use and remain in their possession for the duration of the loan.

1. Purpose of Loan
Devices are intended for:

  • Single or multi-case AAC evaluations.

  • Long-term access within AAC-intensive settings for ongoing client assessments, demonstrations, or clinical use.

2. Terms of Use

  • Devices must remain under the clinician’s care and are not to be sent home with clients.

  • Loan availability is first-come, first-served and subject to Forbes AAC approval.

  • The standard loan term is 4 weeks. Extensions may be granted for AAC-intensive or high-use environments upon request and are subject to approval by Forbes AAC.

  • Clinicians agree to participate in periodic check-ins with a Forbes AAC Assistive Technology Specialist to support appropriate device use and gather feedback.

  • Forbes AAC reserves the right to request the return of any device at any time for any reason. All equipment must be returned promptly upon request or by the due date listed in the order.

  • Late returns may incur fees.

  • Clinicians are responsible for the safe, on-time return of the order. Devices should be returned in good condition with all included accessories unless otherwise noted.


Category 2: Trial Devices for End Users
These devices are provided for client use at home to support the funding process but are only available when initiated by a licensed clinician.

Clinician Role

  • The Clinician agrees to educate and support the End User in the proper use and care of the device.

  • The Clinician will encourage the timely return of the equipment but is not held liable for any device that is lost, damaged, or not returned by the End User.
